This Data Subject Application Text has been prepared to explain how data subjects whose personal data are processed may exercise their rights before the data controller Fatih Kumaşçılık within the scope of the applicable personal data protection legislation.

3. Application Methods

Data subjects may submit their requests to the data controller through one of the following methods:

• Application by hand delivery or postal mail with a wet-signed petition,
• Application through a notary public,
• Application with secure electronic signature or mobile signature,
• Application via registered electronic mail address,
• Application via the email address previously notified to the data controller by the data subject and registered in the data controller’s system.

4. Information Required in the Application

In order for the application to be evaluated properly, it is recommended that the following information be included in the application:

• Name and surname,
• Identity number for citizens, nationality, passport number or identity number for foreigners, if available,
• Residential or workplace address for notification,
• Email address, telephone and fax number, if any,
• Subject of the request,
• Information and documents related to the application.

The data controller may request additional information or documents in order to verify that the application belongs to the data subject. If the application is made through a representative, a document proving the authority to represent the data subject must be attached to the application.

5. Evaluation of Applications

The data controller concludes the requests included in the application as soon as possible and within thirty days at the latest, depending on the nature of the request.

The application is concluded free of charge. However, if the process requires an additional cost, the fee determined under the applicable legislation may be requested.

6. Rejection of the Application

The application may be rejected if the request is contrary to legislation, the identity of the applicant as the data subject cannot be verified, the request is not capable of being fulfilled by the data controller, or for other reasons set forth in the applicable legislation.

If the application is rejected, the reason for rejection is notified to the data subject in writing or electronically.

7. Right to Complaint

If the application is rejected, the response is found insufficient, or no response is given within the required period, the data subject may file a complaint before the competent authority.
